Prior to your intake appointment, there is new patient paperwork to be completed. This can either be downloaded from the right hand side or you may present 15-20 minutes early to complete the paperwork in person. Please note, in order to secure your appointment, we will need confirmation of your payment method such as verification of insurance benefits.
